You wish to subscribe to the t+ service which is operated by t+ Medical Limited and t+ Medical Limited wishes to make the t+ service available to You.

The t+ service (the 'Service') comprises the transmission and storage of health-related information, the analysis of such information and the supply to You and to healthcare professionals of both original data and derived analytical information. Confidentiality and privacy of customers is of the utmost importance to t+ Medical. The storage of data and its access to you and to your nominated healthcare professionals will be provided in a secure manner. t+ Medical fully complies with all aspects of the Data Protection Act 1998. The information that t+ Medical gathers from You is used by t+ Medical to ensure that it can give You the best possible Service.

The Service is designed to assist You in the self-management of medical conditions however it does not give or purport to give medical advice. If you need medical advice, you should seek it from a suitably qualified medical professional.

Before You use the t+ Service You should agree to the TERMS and CONDITIONS (including Data Consent and the grant to you of a software licence for the t+ software). These are set out in full here and if you use this site you accept the limitations that these terms and conditions set out.

If you do not wish to accept these TERMS and CONDITIONS We regret that We are unable to supply the t+ service and you should cease to use this website.

By accepting these TERMS and CONDITIONS you are entering into an Agreement with t+ Medical Limited (registered in England number 4247624, registered address Druces & Attlee, Salisbury House, London Wall, London EC2M 5PS). You should read these TERMS and CONDITIONS carefully before accepting them. The date of the Agreement will be the date on which you accept these TERMS and CONDITIONS.

You are referred to as 'You' in this Agreement. t+ Medical Limited is referred to as 't+ Medical' or 'We/Us' according to context (where such term shall also mean any subsidiary or associate of t+ Medical to which t+ Medical shall assign any or all of its rights and obligations under this Agreement).

Data Consent

Confidentiality and privacy of customers is of the utmost importance to t+ Medical. t+ Medical fully complies with all aspects of the Data Protection Act 1998. The information that t+ Medical gathers from You is used by t+ Medical to ensure that it can give You the best possible service.

You acknowledge that the data that t+ Medical gathers from You is personal information from which You can be identified.

You agree to t+ Medical processing your data, which includes health related information, for the purpose of providing the Service to You.

You agree that your pharmacist and medical professionals may input data on your medical condition and your medication into the Program and the dataset which we keep to provide the Service to You.

You acknowledge that in providing the Service t+ Medical will disclose your data (including information relating to your medical condition and medication) to medical professionals and third parties involved in the provision of the Service, including medical professionals and pharmacies nominated by You for the receipt of such data. t+ Medical will ensure that such third parties are required to keep that data confidential and only process the data for the purposes of providing the Service. You acknowledge that such medical professionals may include your data, including health related information, in your medical records.

You acknowledge that t+ Medical may disclose the data if t+ Medical is compelled to do so by law.

You acknowledge that the data may be included (without identifying individual information) in statistical analyses and for research purposes and that anonymised data, whether aggregated or not, may or may not be used by t+ Medical for promotional purposes.

You agree to t+ Medical accessing your data for internal services administration and maintenance however You require t+ Medical to maintain full confidentiality as to your identity.

t+ Medical will ensure that if it uses contractors, including data processors, such contractors comply with the appropriate standards for the maintenance of external data and that those contractors are under a similar obligation to maintain full confidentiality as to your identity. t+ Medical will ensure that the data cannot be accessed by such contractors save on a need-to-know basis for maintenance purposes only and that it is password protected.

You agree that the data that t+ Medical gathers from You may be transferred outside the European Economic Area but will continue only to be processed in accordance with this policy.

t+ Medical will hold the data for as long as You use the service. You have the right to request a copy of the personal information, including health related information, that t+ Medical holds about you and to have any inaccuracies corrected. If You wish to make such a request please contact the Technical Director of t+ Medical at the head office of t+ Medical as displayed on t+ Medical's website www.tplusmedical.com

t+ Medical may also use carry out a credit assessment of your application, which will involve searching information held about you by licensed credit reference agencies and may retain such information as it requires. t+ Medical may generally process your personal information to manage your account and for marketing and product analysis information.
