Gestational, Type 1 and Type 2 Diabetes Management

The t+ self management system for Gestational, Type 1 and Type 2 diabetes works with most modern mobile telephones. The t+ diabetes system is initiated by a a small software programme which is texted to the patient via SMS for them to easily install on their mobile phones.

The patient's mobile phone will now have an icon on its menu allowing the operation of the t+diabetes health management program. New information is recorded whenever the patient takes a reading or just wishes to update their information. The patient can then obtain key information relevant to their personal management of their diabetes - the system collects data on blood glucose, weight, blood pressure, meal times, exercise and insulin usage and is customisable to collect only information that is relevant to the patient.

At the same time that the patient is reviewing feedback on their mobile phone, it is seamlessly sending information onto a personal secure web page.

The patient can arrange for their GP or Healthcare Professional to access the website and therefore gain access to data such as diaries, charts and reports relating to gestational, type 1 and type 2 diabetes management. This saves time on personal visits thus allowing consultations over the telephone.

t+diabetes also provides the patient with additional reassurance between booked consultations with their nurse or GP - our customer support team in Oxford includes a fully qualified specialist nurse adviser who is available to answer the patient's queries over the telephone or via email.
