Clinical Professionals welcome to t+ Medical
t+ Medical offers Disease Management and Remote Monitoring Solutions that have been designed to improve the clinical outcomes associated with the chronic disease patient.
t+ Medical understand the pressures that can make it difficult for clinical professionals to dedicate as much time as they would wish to patients with long term conditions. The t+ system can assist the clinician by monitoring patients through our specialist nurse service. This means that both clinical professional and patients can receive help and support, ensuring valuable surgery and clinic time is spent with those patients that need help the most.
Additionally the clinician can also view and monitor their patients using their own t+ Medical secure web page that uses proprietary algorithms developed by Oxford University. This can highlight, in just a few clicks, those that need additional help in managing their conditions. With this knowledge, the clinical professional can then prioritise advice, intervene, and potentially prevent an unplanned hospital admission.
The t+ system transforms a patient's mobile phone into a personal health management tool giving;
- A window into how patients are controlling their condition in 'real time'
- Allows clinical professional and patients to share data and interact without necessarily the need for a surgery visit.
- A secure web page to view your patients entered data
- A web based dashboard allows the system to be tailored to individual own requirements.
