Healthcare Manager's welcome to t+ Medical
Long term conditions currently account for 80% of GP visits and with the prevalence of LTC's in the UK expected to reach 30% by 2025 the pressure on GP's time and resource can only increase. New drugs and medical techniques are delivering consistently better results but are often restricted by poor patient compliance.
It is becoming a clear that the current explosion in chronic diseases such as diabetes, COPD and other respiratory complications requires a new model of healthcare provision. t+ Medical believes that this should involve constant vital sign and symptom observation. By linking this to a healthcare monitoring centre it has been clinically proven that exacerbations and emergency admissions can be dramatically reduced. This type of intervention is able to manage over 1,000 patients each compared to around 50 for a conventional community matron and deliver improved patient compliance as well as large scale, cost effective education programmes.
Rudimentary stratification shows that such a system could be optimally employed in over 1.7 million of the UK's 9 million patients with long term conditions and would save the NHS up to £1 billion.
The t+ system harnesses the computing power of the patients own mobile phone and combines it with a fully trained nurse monitoring team using proprietary algorithms developed at Oxford University. This delivers real-time patient monitoring with a low cost, high convenience solution.
By transmitting vital signs information to a secure encrypted web site, the t+ “Tele-nurses” and other healthcare professionals can now evaluate long term condition patients in real time.
The t+ Medical prioritisation process highlights, in just a few clicks, those patients who need assistance with the management of their condition.
t+ Medical has undertaken 18 clinical trials in a wide range of conditions including Asthma, Diabetes, COPD, Cystic Fibrosis and Cancer. For more information on our clinical trial portfolio please contact us using the 'Request Information' tab below.
Effective patient monitoring can provide significant reductions in GP time and unplanned admissions combined and offer better clinical outcomes for patients.
A fully monitored patient programme can be implemented for less than £200 per patient per year
